It is a must at the facility i work at that the CNA's and LPN's attend report. RN's it's optional. The LPNs read the nursing notes from the previous shift and highlight to the CNA's anything extra important. I feel without the CNA's we wouldn't be able to function. Even when we are one or two short all hell breaks loose. Keep up the great work!

I wish I could give my CVA report we try to touch base through out shift but the 11-7 nurses give them report before I get there,,,,, we are a team

I give my CNAs report after we receive ours. It is the only way I know of to make sure nothing gets missed. Then at the end of the shift I expect them to come and give me report on their residents so I have a full report for the on comming shift. I also use report time at the start of the shift to instruct or remind when something comes up.

I use a 24 hour report sheet for my CNAs, and as DON, I read the sheets from both the nurses and CNAs and cross reference them....it helps alot, and I get alot of info from the CNAs that I don't get from the nurses. It forces the nurses to take action on what the CNAs are concerned about, and informs the CNAs on what they need to know about

The nurses and CNAs shifts overlap by 30 minutes, so when I get to work I get the scoop from the night nurse, and then the day nurse after she gets report. The CNAs give report to each other, however it's pretty useless, since all it amounts to is "so and so is 'fine'" when frequently something important is not conveyed like oh, Mr. Jones is supposed to be fasting after I just gave Mr. Jones a big fat waffle. I hate that. Because then the neurotic nurse I work with freaks out and yells at me. So I know to fish for information on my own.

Forgive my ignorance here, what is a 24 hour report sheet?

Loving the unloved....do you work in long term care or hospital? The nurses have a 24 hour report sheet in most facilities. It covers 3 shifts, and the nurses write what is going on with the patients, appointments, new orders, etc. I use the same form for the CNAs to do the same thing. They write down any concerns with patients, changes, open areas, etc. I, as the Director of Nursing, read the CNA report, and the nurse report. I let the CNAs know what they need to know from the nurses report, and let the nurses know what they need to know from the CNA report. I then monitor that everyone's concerns are followed up on. It works GREAT

I work in a NH. We ran into a problem since we work 12s. For instance, a few weeks ago, I found a huge bruise on a resident's hip. I reported it, and neither the nurse or any other aides on the floor knew about it. It was everyone's Monday. The nurse checked his nursing notes and discovered that the resident had fallen about 72 hours earlier, the bruise resulting from that. On the day he fell, it was given in report the day it happened, but for those of us that didn't work those 3 days, we had no idea.
