We were given a case study by our clinical instructor and I really can't figure it out!

Basically, she wants to know why this patient is NOT getting any cardiac meds.

The patient was admitted for atrial septal defect...had surgery for that and mitral valve prolapse. Last strip showed NSR with PVC's.

Vitals looked good, bp on the low side.

Patient is taking ferrous sulfate, protonix, toradol, and lovenox.

I am really lost, if you guys can point me in the right direction, I would appreciate it. Or maybe a clue as to what I should look at.

How low was the BP? I am thinking that if the BP was low enough, they wouldn't want to risk lowering it further with cardiac meds, even though the pt is having PVC's. PVC's in themselves are not always treated with meds if they are only happening occationally. A good idea would be to look at the meds that the pt is already on and see if any of them interact with cardiac medications.

Ok, so we found out more about this patient. The reason the patient didn't have any cardiac meds is because the patient simply didn't need any. Go figure!

That makes sense though. A septum problem is a structure problem, not a problem with the pump. If the patient is perfusing those PVCs, or they are so occassional that they're not causing circulatory issues, shouldn't be a problem. After all, we all throw PVCs!
