Whats the difference how do they interact!

hello to everyone-

i was wanting to know if anyone could please answer this, what is the difference between a health assessment and a care plan, that is if there is a difference. i am in my first semester, about three weeks in! i am just a little confused about these two topics.... do you have any suggestions on books that help me alittle better in writing one up, either or.?

Well where I come from the health assessment leads on to doing the care plan.

A health assessment is the actual assessment you do on the pt. The care plan is where you lay out on paper the care that you are going to provide to the pt. Your health assessment will be included in your care plan as one aspect of care that you will be providing Hope this helps and good luck in school

You health assessment is what you gather about the patient. Their lungs sounds, bowel sounds, skin, what they say, etc. Your care plan is where you take that assessment, and any other pertinent info, and decide what your priorities are for that patient and how you are going to care for them.
