What are your ICU policies?

Hello all! I'm currently working on creating ICU policies that we are lacking. Would anyone be willing to share the frequency of assessment for various conditions (mainly any differences between the vented and nonvented patient)? Any info you are willing to share would be helpful. We have our various drip policies in place. What we are lacking are policies for the operation of a shift (assessment frequency, frequency of trach care, visitation).
