Rating Hand Grasp/Foot Push

I have been saying "Hand grasps strong bilat/foot push strong bilat" ect... on my assessments for nursing school, but now my teacher is asking me how I could say that differently. Is there a number scale? In all my semisters before we have just said "weak,strong,moderate" ect...

I work on an ortho/neuro floor and we have a 1-5 scale that goes 5 for normal strengths, (for feet meaning dorsi and plantar flexion) 4 active against gravity and some resistance, 3 active w/ gravity removed , 2 flicker/trace of movement, 1 no movement.
