I live in MI. I am writing to inquire how to report a facility that is having their nurses utilize presigned photocopies of CII forms.

I'm sorry, I see it now. I initially read "determined 2 ban nurse" which sounded a bit vindicitive to me. Again, I'm sorry,I didn't read it correctly the first time.

I'm hoping that I'm misunderstanding what you're describing, since you are describing a felony. Hard copy prescriptions for controlled substance are required to be on security paper which makes it apparent if it's been photocopied. Photocopying the security paper itself, whether it's been signed or not, to write additional prescriptions is a felony. Each prescription must be written or printed on an original security paper script. This can be faxed to a pharmacy but this is simply a reference for them so that they can have an order ready to go, the hard copy (not a photocopied version) must be presented when picking up the controlled substance.
