So I recently found out that I was placed on the State of Ohio exclusion list. I am not on the federal list just the State of Ohio. It has been 5 years and I was a STNA when this happened and I am now LPN almost done with school for my RN. I was employed and got pulled into the office and terminated because they said I was on this list. I don't know nothing about this or how I even got put on this list. I never received a letter nothing. How do I get my name off this list? Please if anyone can help. I am not able to apply for a job as a nurse in the State of Ohio?

If you are placed on the exclusion list in the State of Ohio not federal can you go to another state and work? Will this show up?

I tried calling and talk to one person and they said in 5 years (which would be in a couple days) I could get it taking off. I called back to get the information needed to do so and now I'm being told it won't ever come off. I called federal and they don't show anything. Just state

Do you have a number. Maybe its a different number then what I have.

I have been so depressed getting sick haven't eating can barley even move. I never in life thought this would happen to me. However I do have my nursing license in Florida. Would this affect me there?

It seems odd that they wouldn't tell you why you were on the list. As far as what the Florida BON might consider or not, that's a tough one. You could look at their application to see what disclosure(s) they require, but there's always a "background check" involved too. I guess anything could come up on a background check just depending on what sort of check they do.

Wow I didn't know I had to go into detail on why I am on the list. Its obviously something I am ashamed of. I came here for help. Not to be ran further down into the ground because I don't tell my story. I have not slept ate or move for that fact. This is something that happened 5 years ago that I paid alot of money to get resolved. Was never told I was going to be put on a exclusion list. Never even heard of it. I have suffered a great deal of embarrassment as it is to let alone have to go through this all over. My lawyer had my record sealed. Thanks for being supportive and helpful.

Where on earth were you run into the ground?

For questions regarding exclusions and suspensions from the Ohio Medicaid program, send an email to "Exclusions" in the subject line and include the individual's full name (including middle initial) and the last four digits of their Social Security number to: [email protected].

You made it sound like you had no idea what happened?? The first step, in that case, would be finding out.

If you already know what happened and want to know what the Florida BON's reaction might be to it, you're probably going to have to ask them. Maybe you could call anonymously before formally applying for a license?

To the OP:

You don't have to tell us, but from your OP and subsequent responses you made it sound like you somehow ended up on this list and had absolutely no idea why. Again, you don't have to tell us, but how are we to know?
