In my facility (nursing home) we don't write order to turn every two hours. The order is to Encourage turn and reposition every two hours. Who I am encouraging an immobile patient? Is this wrong?

I'm unsure of the question. I think "encourage" basically means explain risks and benefits before you turn them so they have a chance to refuse, of course with emphasis on risks. Which we do anyway for anything. what matters most is that the patients don't get pressure ulcers, and document if they were turned , and if they refused "risks and benefits explained" or any situation in between. (Like it's not always feasible to just explain risks and benefits if the person is demented or agitated etc but there are other things you can do to encourage) If they're independent then just education about pressure ulcers and reminders daily. In other words , this order should cover it overall.

An immobile pt would be care-planned with interventions for 'freq turning and repositioning by staff'.
