Assisted Living assessments in Connecticut

Can anyone please assist me with the types of assessments that have to be done in assisted living communities in Connecticut. I called the DPH but they didnt give me much help, but advised me to look up the regulation. I did look up the regulation, but was not very helpful. I have specific questions.

I did find out that there is no standardized assessment that every community uses their own, but it sounds like it needs to have certain requirements/info captured on the assessment. Here are my questions below, if any nurse can please assist.

1. What types of assessments are done in CT in Assisted Living facility? Admission, service plan, etc?

2. What are the frequency of the assessments/care plans?

3. Any recommendations where I could get an example/pic?
