"Find a patient on the floor from a suspected fall". (select all the apply)(no other information ie injuries)Does that necessitate a call to the provider? or no harm no foul?

A fall would be considered a change in condition, even if the patient can walk it off afterward. As nurses, we can only assess, decide what we think is wrong to advise the physician, and then follow through with notifying them.
