Hello,
My name is Howie....and I'm a Senior nursing student, and a long time member of allnurses.com....
I was wondering if anyone here would mind me running some questions by the forum.... ? My professor gave an assignment with quite a few NCLEX-like questions.....Some of my classmates, and I, have read our Med-Surg inside and out, and, as is custom with NCLEX questions, wev'e narrowed a few questions down to 2 or 3 answers.....
I don't want it to seem like wer'e trying to get the work done for us....wev'e worked very hard....done a ton of reading....and have come up with rationales for why we would pick some of the remaining answers.....
Would anyone here mind if I ran a few questions by the forum? I'm more concerned with the learning experience.....Wer'e just Type-A, OCD nursing students, and we really want to do the best we can on whatever assignments we are given.....

Wev'e made it through the assignment pretty successfully so far, with this exception of just a few, where wev'e left hashmarks, and question marks, next to the options wev'e narrowed the questions down to....
how about THIS one to start....
The client tells the nurse that upon waking this morning, the vision in her left eye has decreased to about one fourth of the area she could see the day before. What is the nurse's first priority?
A) Ask the client what prescription and over-the-counter meds she is currently taking.
B) Place the client in the supine position and patch the left eye
C) Document the finding as the only action.
D) Notify the physician.
....Typically, I'd lean toward an answer, other than 'notify the physician', except in certain situations, but, for this one wev'e narrowed it down to A) and B).....I personally think 'notify the physician' would be the first priority in this particulare situation.....any ideas?

I would "notify the physician"
Notify the physcian. It's a change. Any change in the patient should be made aware to the doc. Asking about the patient's meds are NOT a priority.

how about THIS one....
A nurse should teach a client who is receiving cycloplegic drug therapy to do which of the following?
A) Wear sunglassses and do not drive or operate machinery until the drug wears off.
B) Use at least a 30 SPF sunscreen agent when going outdoors
C) Avoid driving or operating hazardous machinery until the drug wears off
D) Turn up the lights indoors because visual acuity will be decreased in low-light.

I'd go with A. You'd want this info when you talked to the MD & to document in the records.
The vision problem could be side effect of a drug. I did a quick web search and sudden vision loss appears to be a potential, though uncommon, side effect of Viagra.
Oh!
I see the others went with notify MD - and that makes sense, too. Especially with tricky way NCLEX questions tend to work... what's highest priority versus what would you do next... so glad to be out of school!
