i just need to vent. every day when i get back to the office, there is a stack of paper work for me to correct.:angryfire he i am doing my visits, and my paperwork, and im thinking ok i answered everything like im supposed to so maybe i wont get this one back. well guess what, the next day when i get back to the office, papers on my desk waiting to be corrected. my heart just sinks and i want to quit. it takes forever to do the corrections and i often have to stay late to finish. not only do i have to finish the corrections, i still have to finish the paper work for the day. sometimes that includes evals, recerts, d/c. im very frustrated. sometimes i think about going back to the hospital. then i remember that my boss told me that it takes about six months to a year to learn the paper work. is this happening to anyone eles or is it just me that cant catch on. i have been in hh for two months. is there hope:o . i dont want to give up yet.

I've been in HH since April 26, 06. Don't give up. When I first started it would take me HOURS to fill out an OASIS packet. Now I can get through one in about an hour most of the time. Paperwork is my strong point but I'm not bragging because I envy the nurses who can waltz right into a situation and can do skills like nobody's business (maybe it is my own fault because I spent most of my nursing years in a low skill nursing home) but all the same, I get real nervous when I'm sent out to do a blood draw or heaven forbid access a port-a-cath.
I still occasionally get an OASIS form back to make a correction, no matter how good you get there will be things you will miss once in awhile...there are sooo many little boxes to check and they still get onto me because I don't fill out the orders the way they want.

Don't give up. And do you understand the reason you are having to make corrections? The reason I ask is that sometimes people who review our work may be just too picky and just have a difference of opinion. Remeber that you are the one who did the assessment and you are a professional, make sure they aren't having you "correct" things that are not mistakes.
I worked for a lady once who didn't like the way I worded my orders & goals and had me rewrite every single one I ever turned in. Come to find out, she was the one in the wrong.

As a OBQI person, the corrections are sometimes needed to clarify a conflict in the documentation, for clairifying the focus of care, for getting the max amount you can for reimbursement, for coding etc.
My first year in home health I learned to hate yellow sticky notes.

well I have been in hh 2 mos. and just got my second write up for paperwork not being accurate. i dont know what to do. i really like my job. i would be very disappointed if i get canned. maybe i am rushing to finish the paperwork on time. but it seems like there is a rule for every thing you write. . if anyone has any suggetstions that would be great. i really want to keep this job.
Why are they being so harsh on your after only two months?
I can't imagine what kind of bosses you have. Sounds like they need to be taken aside and be shown a thing or two.
The new DON I work under is wonderful in that she always sticks up for the nurses. I heard the office manager on a high horse complaining about a nurse (could have been me, for all I know) who "still hadn't figured the OASIS forms yet, blah blah blah" and I heard the DON say, "well, she's having to learn as she goes along, like you did!"
I totally agree with Motorcycle mama. It took me at LEAST 6 months to quit getting every single paper I filled out back for corrections.
Maybe you should go back to the person who wrote you up and express your concerns. Maybe you could suggest some more orientation to the paperwork??
I hope you hang in there, cause once you get it figured out, home health is the best!!

SHewwww, the hated paperwork. It's bad enough to have SO much to do, then to get "audited" on what took so long to begin with. The HH im at now doesnt use computerized cahrting like im used to and there are 2 nurses the "check" our notes and we have to make corrections. All fine and dandy but when you ahve to go find a chart(that only God knows where it might be) then find the dated note that you are being audited on, only to find out that the "audit" is about not drawing a line through something????? VERY frustrating!!

I'm glad to hear I'm not the only one dreading yellow sticky notes. Some days I just want to scream because I have so many corrections to make. Today I had none in my box....yeah!!!!

Sticky notes... yeh buddy ! Got BUNCHES of them back the first few months.. now I've got it down pretty good.. know just what they're looking for and what to do/not to do. But even so.. I still get some back now and then. But that's ok.. I don't see it as "punitive" or judgemental.. nor do those who check them make us feel that way, either...they just need to have them correct for any MCR auditor that might walk through the door, and I understand that. So to me, it's really more just "finetuning" and I expect to get some back regardless. No big deal. We all do. We correct them and then they can be put in the chart.
No one at our agency has EVER fussed at us for our needed corrections.
I can't believe you were written up for this??? Sheeeeeeeeeeeeeesh !
Home Health is awesome... hang in there. But it sounds like they are being a bit uptight and overly judgemental... as long as it gets corrected, who cares? The focus should be PATIENTS FIRST.

Hang in there.... It takes a long time. I worked for a small hh agency once that bragged about being all computerized. WHAT A JOKE... I was constantly going back and reentering and reprinting... And I had 6 years of home care experience to boot.. The person at that small hh agency had the gaul to ruddly tell me..."I thought you knew oasis.. you would never know it by your documentation"... This was coming from a nurse who had only been a nurse for less time than I had been in home care. ( I had been a nurse for 13 years prior to that). I gently reminded her that I was a practicing nurse when she was still driving her kids around to soccer games. Her pressure on me got really worse after that and I quit soon after. HANG IN THERE... IT GETS BETTER... Just think... your manager could start hiring managers that have no home care experience... :angryfire teeheee...... thats another story for another time...... HANG IN THERE..It's worth it....:monkeydance:

i have to say, i am one of the nurses that sends back the notes for corrections, and believe me i really don't like those "sticky" notes any more than you do. please don't take it personal...but remember to give yourself credit for all you do, and if it isn't documented it wasn't done. so hang in there, ya'll doing a great job.
