Copy of CA RN license

I've noticed that some applications (school nurse application, preliminary school nurse credential) ask for a photocopy of a valid registered nurse license. I am licensed in California and they no longer give out physical cards of our nursing license. How do I obtain one? Do you go to Breeze and choose verification letter? Wondering if that's correct because it does not ask for my mailing address.

You can electronically print out license form @ https://search.dca.ca.gov/ to include with an application.

Facility will use that to verify license information.
