Case managment??

What is case management? I have read a few threads but nothing really explains it. Thanks.

The Commission for Case Management Certification (CCMC) defines Case Management as:

"The practice of case management is a process that manages client wellness and autonomy through advocacy, communication, education, and the identification and facilitation of services".

I'm an ER case manager. My duties fall into three different categories: Utilization review - where it is decided if a patient meets medicare-directed criteria for admission or observation, discharge planning (the fun stuff) where I try to find appropriate resources for pts going home and true case management - where you manage chronic patient's care.

Thanks for the info! I was just curious. I am a bedside nurse but I have been wondering if I want to do something else and it was a curiosity for me. Blessings. Lori RN
