careplans for the resident whose family doesn't attend and hardly visit

Was just curious to see what other facilities currently do when family members of a resident choose not to attend careplans and they do not visit often. How do you document to prove that you do in fact invite the families to careplans? I work in a dementia unit and most of our residents do not attend their careplans. Someone suggested keeping a copy of the careplan invite we mail to the families. Another suggestion was to mail a copy of the careplan to the family.

We keep a copy of the invitation we send to the families for the care plan meetings. At the top of our care plan summary, there is a space to write the name and relationship of the person attending the care plan meeting. When nobody from the family attends, we simply write, "family declined invitation." There is no need to send a copy of the care plan to the family, unless they request it. We have also done care plan meetings by phone for families who live a distance, are elderly, or cannot attend for other reasons. We have pretty good success with that as well.

We do the same thing. Invitations are sent and kept in the chart. If the family doesn't attend, the notes reflect that. I encourage all residents to attend since it is THEIR meeting, but most of them don't want to be bothered.
