For those who were nurses for Nurse family partnership or currently are, what are some career options afterwards? Besides being a supervisor what else can you do in the maternal/ chills home visit field? Thanks.

I'm not familiar with that particular job title, but I'm assuming it is a type of transition role ; ensuring patients' care after discharge?? If so, you may be in luck.
Based on my increasing exposure & involvement in the new health care models that are part of Health Care Reform, there is going to be more and more emphasis on making sure that the transition between different levels of care is smooth and effective... if you're organization is part of an Accountable Care Organization or "Medical Home", this will be crucial. As a result, organizations are already creating 'transition nurse' jobs. These are nurses who can go back and forth across the acute care home care boundaries to improve the effectiveness of care and reduce unnecessary re-admissions. From what I understand, the current roles are like Case Management, except they don't deal with the financial stuff, just clinical coordination. Keep your eyes open for these opportunities.
