Hey guys,

I am working on a care plan, and have a few ideas as far as the nursing process and goals vs outcomes. What I'm wondering is if anyone has a good resource that has really good samples of a mini care plan that is short and sweet.

Of even if anyone has examples of good evaluation writing that would be a great help.

It depends on what your pts DX is-

This patient has benn admitted due to a sudden onset of right sided hemiplegia

He has a history of non-insulin dependant diabetes mell and hypertension.

After 48 hrs his speach returns but now has trouble swallowing thin liguids and food, but is alert to time place and people. However his right arm has gross uncontrolled movements and has begun to experionce incontinence.

I have identified the problems, and have set the goals and approaches, so all I need now is examples how to write a decent evaluation to summarize for each diagnosis.
