care plan help

i am confused by my first care plan.

we have to prioritize our diagnosis based on their importance and then list the area of assessment and nursing diagnosis

so for dementia i am just confused at what areas of assessment would be?

so far i wrote:

performance of ADL's

i just dont really know what is acceptable to write..

help?

Think along the lines neurological, cognitive, and behavioral assessment. I'm pretty sure it's in Lippnicott, nursing guides, and other sources.

Impaired phyiscal mobility r/t nueromuscular impairment ?
