Does anyone have a specific form that the use when creating care plans? I did find one for food allergy. But I am struggling to find something for diabetes and asthma.

I'm a self professed under achiever but I seldom create a care plan. I use whatever information I receive from the physician's office. It may take on the form of a formalized Diabetes Management Plan, Instructions written on a fax, or it may simply be the prescription label on an epi-pen. ?
My big issue is that we have to bring an RN to complete my care plans (LPN's cannot write them in Fl) and we are not allowed to use our district forms.

yep - I only write/create them if asked.

We have to have them for those with 504's if they also have a health condition.

Even then - I only do them if I actually see the kid on a regular basis.
We have several kids with ADD, Asthma, allergies etc that have 504's and accommodations for medical have been included in their education plans - but I never see the kids nor do I have any medications for them in the clinic.
I try my best to stay far away from writing care plans ?
