    We say less than 2.0

    You should have an institutional policy/procedure for removal of pacing wires which dictates this. We have INR < 1.5. I did recently see cardiac tamponade at postop day 15 when wires were pulled with INR 2.0. Also depends on the patient; some AF patients have a higher risk with low INR so they'll do it with INR ~2.0 then watch them.

    We don't have a policy in my hospital as often times we are "needing" the INR to be therapeutic in patients who are S/P Valve replacements, or have had A-fib, nor has any other facility I've ever worked at. I've rarely seen an issue with d/c'ing the wires, and never seen a tamponade that was related to wire removal in nearly 11yrs.
