In NCLEX world, LPNs can do follow-up assessments but not initial or focused assessments. As for the real world, I don't know. I do know that when I worked in LTC, the LPNs did it all. They were awesome.

In my state, LPNs cannot assess. It really depends on your scope of practice in your state.

In my state, LPNs cannot do comprehensive assessments, though focused assessments are entirely within scope. What you are describing would not be in the scope of the LPN in my state (Texas).

It does vary by state. My son is an LPN in a nursing home, and he does admission assessments and initial care plans everyday. LPNs in my state (OR) can do almost everything but insert PICC lines, hang blood or give drugs via IV push.
