I passed my NCLEX last week and called the BON to find out the status of my license. I was told that the NJBON now requires candidates to send a copy of their birth certificate with proof of citizenship before the license # can be issued. Has anyone else heard of this? Thanks!

From the NJBoN application:
PLEASE SUBMIT THE FOLLOWING TO THE NEW JERSEY BOARD OF NURSING:* The Official Application for Licensure by Examination;* Attach a clear, full-face passport photograph (2 ̋x 2 ̋) of your head and shoulders, taken within the past six months, with your name printed on the back of the photo;* A copy of your birth certificate;* The total fee of $200.00 made payable to the New Jersey Board of Nursing (a money order or personal check is acceptable); and* The Certification and Authorization form.

If you are born in the US this proves your citizenship. Per federal mandate only US citizens and legally authorized aliens are to be issued a professional license (nursing or any other profession such as accounting)

I guess Indiana has the honor system since I just checked the "Are you a US citizen?" box, they didn't ask to submit a birth certificate.

I am a citizen and I checked that box as well! Just didn't see that they required a birth certificate.
