Bedside Report Sheet

The MICU that I work in is currently trying to come up with bedside report sheets that are simple yet fill the nurse in on the important face about the patient. We want it to be an easy way to tell when the patient was intubated, extubated, what procedures were done on what day, etc. Trying to research what others are using out there in their ICUs. Thanks!

I don't have a copy of ours but we keep it in a binder, one for each room. It has a few pockets that we keep the sheet in, which documents stuff that is helpful for a pt who has been here a long time. Were their core measures addressed? Vaccines? When was the foley put in? etc. I found it's also a nice place to keep frequently-used papers, like insulin drip flowsheets/protocols. :-)
