Hi, In a home care setting I was giving to take care of someone else's pt. The nurse was giving him SQ injection of B12 30-60 min prior to him going to hemodialysis. Could it be wrong? Does anyone know B12 clearance rate during the

hemodyalisis TM?

Thanks a lot. Just trying to do the right thing for the pt.

I don't give any vitamins before dialysis. Vit B12 is water soluble and will be dialyzed out. Just my opinion.

I've always given nephrocaps before HD.

Nephrocaps are water soluble, so they will just be dialyzed out.

Been giving them for 20 years on the advice of several nephrologists around the country. Dialyzablitiy is based on molecular weight isn't it?

If giving IM or SQ it can be given within the last hour of treatment same as we do with antibiotics during treatment. By the time it's absorbed the pt is off dialysis. I generally like to give it just before pt is coming off as it only takes a minute or 2 to do. If giving at home then give on a non-dialysis day or after treatment.

I think the original post was in question to giving SQ injection PRIOR to going to dialysis, by a home care nurse.

The extent to which a drug is affected by dialysis is determined primarily by several physicochemical characteristics of the drug.

These include molecular size, protein binding, volume of distribution, water solubility, and plasma clearance.
