? to ask when you become Psychiatric Advance Directive (PAD)?

I am senior level nursing student and starting my psych classes in a few weeks. I have been put in a position to become a family members Psychiatric Advance Directive (PAD). This person has been diagnosed having paranoid schizophrenia. I want to be sure I am asking all the questions that I should. Does anyone have questions I should include?

I'm not entirely sure what you are,looking for, but NAMI has some excellent information re PADs.

However, you'll be acting as this persons agent, so you need to talk to the person and pin down exactly what they are expecting from you.
