Hi there, Im a second year nursing student looking for help with a patient drug profile assignment. The purpose of this assignment is to create a profile on a patientand the 4 prescribed medication that they are currently taking. Unfortunately I am having a difficult time finding someone who is taking at least 4 medications. The required information is patient demographics (age, culture, occupation, and relevant health history) and information regarding the illness/chronic health challenge that the prescriptions are for. Additionally I need the name, dosage, and other information regarding each of the 4 medications.

EryPed 200mg/mL 280mg QID (gastric motility)

Qvar 80mcg 1 puff BID daily maintenance (prophylactic asthma) increase to 2 puffs TID x7 d for flare

Ventolin HFA 90mcg 2 puffs q4 h for wheeze, SOB, cough, increased work of breathing and 2puffs 10-15 minutes before increased activity, sports , PE class

Bupropion 75mg tablets give 2 tablets (150mg) BID (2nd dose no later than 5pm to reduce potential insomnia) for anxiety, improved attention r/t autism and ADHD-inattentive

Oxytrol 3.9mg/ day patch apply one patch to hip/lower abdomen 2x week (every 3-4 days) rotating site (bladder spasms/dysfunction)

Questran 1 pkt qday 2 hrs before any other medications

I will let you try to figure the why in terms of why those medications are prescribed based on the DX
