I have to do a careplan on and my patient had hydronephrosis, I found a great care plan in Lippincotts manual and now my care plan book at home doesn't even come close. I have been all over looking on Ebooks and can't seem to get it downloaded. I could use your help. I need a page number for reference, altered elimination as my nursing dx for this.
