Any Disease Management Nurses

I looked for a topic for this type of nurse. It is really not case management as the program I have looked at is an elective program for an insurance holder to try to educate them to keep their health care costs down. I am enrolled in one myself for asthma and like the extra information I get on latest this and that. I was not in the initial stages of the program long and do not have a disease manager anymore but really thought it was a good idea.

Does anyone do that type of nursing here ?

Has anyone been in the program as a client and did you like it?
