I have accepted a new job and will not be returning to original job after my maternity leave. How should I go about resigning? Should I email a resignation letter to my manager and call her to tell her over the phone as well? Do I set up an in-person meeting to tell her? I do not want to continue working there but I do want to maintain a good relationship. Thoughts welcome.

Can you just let them know now that you've decided to stay home with the baby? Maybe it's not necessary to tell them you are changing jobs?

You might need to be careful about your benefits and things if you're not coming back off maternity leave. I know some places will charge you for your insurance that they paid on your behalf while you were out and things like that, so if you have somebody that you trust you can talk to or policies you can review that might not be a bad idea... Just an FYI more than anything bc I have a friend that faced this at my last job.

I agree find out where you stand with benefit issues. Either way you are not going back and you won't be surprised if you missed something. If its major you can get some preparation plans started to deal with it.
If you had a good relationship with your manager, I would recommend speaking with him or her. Simply express that the new opportunity is beneficial for you and that you appreciate having worked there. If not or just borderline just follow the resignation policy. I don't think you have to be concerned about a bad exit if you simply provide notice expected.
