If PRN staff have medical insurance through their spouse's employer, can they sign a waiver and work more than the 30 hours per week?

That depends on your employment agreement and how your workplace uses PRN staff. Most prn jobs do not offer benefits. They figure that you have worked out your own insurance and retirement either through your spouse, primary job or your own funded plans. Where I work, prn staff only have minimum hours to meet. For example the minimum here is one 12 hour shift in a pay period. I have never heard of upper limits.You would probably need to ask HR about that.

Your employer is free to have you sign a waiver and give you extra wages in lieu of health insurance. Each employer creates their own waiver, here is an example: Michigan Business & Professional Association > Procs > Business Knowledge > Healthcare Reform Connect > Expert Advice
That doesn't mean however that an employer would necessarily want to let you opt out. If you're working that much it might be more to their advantage to include you in the plan, since the more employees they have on the plan the cheaper it is per employee.

This is completely up to the employer. The ACA does not say that employers cannot allow their prn staff to work more than 30 hours a week, just that they must offer those prn employees benefits if they do work more than 30 hours a week. Employers have taken it upon themselves to make restrictions on the maximum hours prn employees can work in order to avoid being forced to make the offer of benefits to their prn employees.
