Good morning- I shared this with another nurse that thought it would be helpful to use in the future. So I thought I would pass it along to you all, as well.
For students who don't do well on the Sports Physical eye screening. (These are usually the ones that we have done everything in our power to get the parent(s) to take them to the Eye Dr.) For one student- I had tried for many years to get mom to take him to the Eye Dr. The student had even come to school recently and announced that "mom said she is not taking me to the eye doctor!!"
When the examiner makes a notation on the Sports Physical Form- something like this- "needs Optometry Evaluation".
I was able to get parents to take 2 kids to the eye Dr. by sending a note from me to explain and to "clear up any confusion about his/her Sports Physical" - that the student needs an Eye Exam. To bring a note back to school as proof that the student had an Eye Exam and I would attach it to the Sports Physical. This would meet the requirements to make him/her eligible to participate in sports.

When it will jeopardize their child's ability to play sports, suddenly getting appointments (including the physical that yes I absolutely need) become so much easier...
I should have done that so many years ago, but just learned its power about 2.5 years ago myself.

Absolutely! I just wish I had figured it out a LONNNNG time ago!

I have done this with Asthma plans, since I seem to have a lot of troubles getting them returned. "In order to complete the sports physical, the student must have a completed asthma plan". As long as the doc/PA is on board with including a note stating this information is needed, it works! ("Student needs Asthma plan"
