Need Help with Medicare Question

I am doing some research for the APN I am following for clinical. I need to know if there is specific criteria for charting to get the max reimbursement from medicare. She is wanting to get all 3 of the Doctors and NP with the same charting sheets for their patients. Help if you can. Thanks for your time. ~K~
