    Hi everyone!
I am a long time reader but this is my first post. I am looking for any suggestions you may have on developing a care plan/emergency plan for a pre-schooler with Demosier's Syndrome. She requires an IM emergency injection of Solu-Cortef due to adrenal insufficiency. Are there any templates out there or web-sites you could direct me to? Also having a tough time finding anything on the syndrome period. Thanks in advance for your assistance! This one has me stumped!

  4. Individualized Healthcare Plans for the school nurse has a pretty good CD as far as a general template with a big heavy manual goes. The book is pretty useful in helping you find the right wording and verbiage for your nursing diagnoses.
Since this is such a specialized issue you may want to contact that student's physicial directly to get fodder for your plan. (i searched too - probably didn't find anythnig different from you)

Found a great website link: http://www.aea267.k12.ia.us/schoolnu...?page=sn_forms which has a nice digital template of ihp's and ep's and so forth.

http://www.schoolhealthservicesny.com/tool_kit.cfm is a great site as well.
  5. I second the suggestion of contacting the child's parents and healthcare providers for information.

I had a student last year with a very rare syndrome that is usually fatal early in life. I could find next to nothing on it, let alone in relation to a 10 year old child.

I called Mom and she was a wealth of information and very gracious in teaching me what I needed to know to form a care plan. She was so grateful that it was being done. The previous nurse had simply not bothered since she knew so little about the condition.
  6. Great resource/websites Flare...thanks for sharing!
