Help me prepare my resignation?

My district has been great to me so I really want to leave on the best terms possible. They already know that I am training for my new position (in skilled nursing) and have allowed me some flexibility in scheduling to accomodate that. Initially, I had planned to work on call at my new job while continuing to work at the school for the remainder of the school year. Well, I have found out that a full time position may become available much sooner than this so I have decided not to return after spring break. If I submit my resignation Monday then this will give the school 4 weeks notice.

I would like a little help wording and formatting my letter of resignation. There are a few points I want to address:

1) my letter needs to go to two specific people - our district RN and our HR director. I'm not sure how to professionally address the letter to both, or do I do two different letters?

2) I want my last day of employment to be 4/18 (technically this is a vacation day - last day of SB)

3) I would like to make myself available on an on call basis from 4/18-5/31 in order to help orient/train my replacement. We are in a very rural area and I know it is difficult to find an RN willing to commute here for low pay and minimal benefits. It's very important to me not to leave the school high and dry, but it's also important that I make my new job a priority after 4/18.

4) Aside from expressing my gratitude for the opportunity to work and the experience I've gained while here, what else do I say?

I really appreciate any help/guidance you can lend with this. I'm fairly eloquent and able to express myself well in writing, but I really could use a hand getting started on this.

To be honest, it is doubtful that anything you write will make much of an impact except that there will now be an open position to be filled. My advice would be to keep the letter of resignation short & sweet, and business-like. Anything else could be sent in a personal note to each of your supervisors.

At least, that's MY take on it! Anyone else want to weigh in on this???
