Utilization Review /Clinical Record Review

I am interested in conducting utilization (or clinical record) review activities for the IRF where I work. I have not been able to find a forms template that would be specific for inpatient rehab and I was hoping that someone might be able to either direct me to a website that may have what I'm looking for or be willing to share one that is currently in use at their facility. Any info would be great. Thanks!
