Is there anyone out there that works LTC in Nursing Rehab?

Anyone who is willing to start a dialogue and get some real ideas on this crazy QIS survey process and anything else we can think of? Including where the HECK to get information on LTC Nursing Rehabilitation issues?

ANYONE?

Hi! I used to work full time in a LTC/ rehab/ SNF and I have my share of QIs and the process is really detailed and it involves a lot of determination and patience to get things done so when survey comes ...you got your stuff ready...no TAGS....! LTC rehab issues... well i'm sure your facility has a big book on what surveyors will look for such as the Ftags and Quality indicators also. Im sure your DON or Administrator should have list. One biggie specially during summer months is Dehydration... another is Wound care...environment... policies and procedures regarding care of your patients. Whatever assessments your RAC or MDS person submitted to the state as a quarterly basis will reflect what kind of care your patients need and surveyors already know what to look for even before they come in your building. It is very important that all the nurses/ cnas and interdisciplinary team ( social worker, dietician , activities and therapy) accurately document their assessment on the chart . Review your policy on medication administration... are the nurses doing what is expected? REhab issues... ADLs- are your CNAs documenting in chart that they are doing their bladder and bowel program; are they recording their ROM and ADL activities accurately? QI is basically a determinant as to how far or how good of a care does your facility staff render to your patients in the facility. hope I somehow answer your question.

I am a brand new grad and the only job I could find is with a staffing agency that has me working at a long term care facility. My orientation was 4hrs for the facility and 1 night for the floor. Gotta say, I hate feeling like I have been tossed out and told not to sink. I have no real experience with vented patients or patients on G-tube or J-tube feedings (other than the quick training in the nursing lab). Nor have I ever had more then 5 patients at a time. Now I have at minimum 14 patients. Any advice. Last night I felt like crying and quitting at least 2 times. I need this job and want to succeed. I hate feeling incompetent. Any advice on how best to get thru this? Did I mention, I HATE PAPER CHARTING!! LoL

Help,

New grad hoping to not SINK!
