Utilization Review/Clinical Record Review

I've posted this question on the Rehab Nursing forum and have not gotten any replies so I'm giving this forum a try. I work for an inpatient rehab facility. I'm responsible for the QI activities and am interested in conducting clinical record reviews but have not found a form that I can modify to fit my needs. Does anyone have a UR form or chart review form that they'd be willing to share? Or can someone refer me to a good website that has templates or sample forms? Any help would be much appreciated. Thanks
