I am googling like crazy, but cannot seem to get anywhere!
When using chart audits, evaluation tools, etc, it was always drilled into my head that we should not use specific patient identifiers such as their names. Chart numbers/patient IDs were okay.
It's my belief that although they are used intra-agency only, there is still the chance for personal and protected information to be inadvertantly released &/or shared.
Does anyone have any input? Does your agency use names on chart audits?
