Input-from hospital to clinic

I am looking for comments from community/public health nurses who have worked in a hospital and then started to work in a community or clinic setting.

I am starting a new position at a clinic after working in a hospital for almost 6 years and i am very nervous about the transition.

Any advice to help me with the transition would be greatly appreciated.
