i have been assigned to be a named nurse to a patient with challenging and physical aggressive behaviour. can anyone help me to design a care plan for this patient.

As we often say here....what do you have so far? We can't do your homework for you (and shouldn't since you won't learn that way), and would need alot more information than you've given us anyway... You need to learn more about the patient than you've written so far.
