Mental Health Status tick-charting?

I am wondering if anyone has come across a charting template for recording your mental health status exam on your patients. We write all ours long hand, and it's very time consuming. We're looking at developing our own for our unit. Is anyone doing EMR on this? Or have an example of tick charting? Much appreciated!
