post-registrattion form from NMC

hello!
i just want to know who will fill-out the POST-REGISTRATION FORM from NMC (Nursing & Midwifery Council). Is it just the applicant or the nursing supervisor of the hospital where the applicant works? also i'd like to ask who will send it back to the NMC.

I understand this is one of the requirements for overseas nurses who want to register in NMC.
