License Card for Nurses


Good day. I just wanted to ask about the requirements needed to apply for the license card? It's for my sister, she recently passed the PNLE. Thanks in advance.

I think you need certified copies of the following: board rating & board certificate. Bring also a recent 1x1 ID photo then go to your nearest PRC branch in your area. If it's for your sister, I think they also need an SPA.

I forgot to mention the birth certificate. They also require that one.

i already applied for my license here are the requirements:

2x2 pic with name tag
1x1 pic with name tag
short brown envelope
and the form that she will be getting " The panunumpa ng profesyonal" form at the prc

You can check PRC's website and download the Oath Form or "Panunumpa ng Propesyonal". There is an instruction at the back of the form. Personal appearance for the initial registration is required.
