im not sure about your situation kathy, but ill tell you mine...i had to do a name change since NSO didnt reflect my correct current name. My last name was changed when i was 5 yrs old. anyhow, NSO wanted a certificate of finality issued from the court of the judge who changed my name...i got that issued, sent it to the NSO's main office and in compliance, NSO added a note below the BC about the judge's order declaring my correct name.

what i mean to say is that if you've exhausted all other options, then NSO will submit to a judge's ruling about what a person's name actually is.

If you dont get it corrected, you might get in trouble in situations that require exact similarity between the BC and other documents...like licenses, FBI records, SS numbers..etc.

perhaps it will help if you can request the NSO to double-check its records considering that the original certificate is old. i think NSO main office has the proper office or officer/specialist who can scrutinize the old paper and give you an opinion/advice. you can also bring a formal letter stating your request for clarification and attach a copy of the certificate, but have time first to verbally explain to the officer what seems to be missing in that document.

Also, I understand that the name in the passport can be amended by DFA upon request if there is a supporting document from NSO.

thanks a lot ethan and kiko.. my mom will go to NSO tomorrow and talk to an officer there about the situation.. hope it will be fixed tomorrow without going to any court..many thanks..
