did the school missed it?

i have another question, i photocopied the 3 forms before the school mailed to the BON. i read it and im now confuse if the school missed to fill it or just right.. the nursing program verification is just filled with my name and by the dean but the questions regarding "describe the format of examination adminstered...etc" leave blank...?
next..about the breakdown of educational... why is it there nothing about course description attaching to the form?in overall, the 3 pages/forms i submitted to the school,also the 3pages/forms +tor.diploma,rle mailed to the BON...
