documentation - need ideas

Hi i am a paediatric nurse working on a 24 bed medical surgical & orthopedic unti. We are concerned that we arew not doing very well on our documentation key performance indicators ansd wonder if anyone has any ideas on how we can improve this - has anyone done anything in their area to improve documentation?
