Parish survey

Hello!
For several months I have been working toward the goal of beginning a volunteer parish nurse program in my parish. Today I got the go ahead and I am very pleased about it.
My parish is a large midwestern Catholic parish and I am wondering if a parish survey
is a good idea and if so, what questions should be asked.
