parish nursing help

Hello- I am a senior nursing student and for a senior project, we have to do a report on parish nursing. we are working with a church and we need to do an assessment of the congregation. We need a good assessment tool to look at to model ours after. Our topic is "healthy heart, mind and soul." If anyone has any suggestions or can give me any good references, I would appreciate it.
