Policy On Narcotics For Acute Care?

I am hoping someone can help me find different facility's policies on the use of narcotics in the acute care hospital setting. PCA use, administration records, assessment and reassessment of effects. Specifically, how do you assess a patients response to the drug? How often are vital signs taken if the patient is receiving a continuous drip? Do you have a witness when setting the pump?
