Transporting patients post MAC anesthesic

Who transports with the patient post procedure (OR, PACU and Endo) following MAC anesthesia?

RN Only
RN and Transporter
Transporter Only

If anyone other than the RN, how is hand of communication handled and documented?

If possible, will you share your policy/procedure or guidelines along with your documentation tool?
